Before a minor is allowed to take part in the study group. The minor needs to return a signed parental consent form. Just copy & paste the consent form into a word processing program. Have a parent or legal guardian sign it. Then either email it to Danielle_0_7@hotmail.com as an attachment or fax it to me. If you want to fax it, let me know so we can set a date and time.

We also agree that The Wiccans Way is not responsible for our minor’s actions outside the class room and that they claim no legal responsibility for the minor(s). Our minor(s) will abide by all rules within the forums and any associated content The Wiccans Way reserves the right to remove the minor(s) from the class if they fail to abide by the rules set by The Wiccans Way. Furthermore we the legal guardians approve all associated materials used within the class. By signing this form you are making the claim that by law you are the minor(s) legal guardian within your legal state of residence. The Wiccans Way is not responsible if this form in any way has been falsified and may reject this or any other form thought to be falsified. This form will become invalid if the legal guardians move from one state to another and another form must be signed for that state.
